New to Yale Health?
For new members, these are the most important first steps:

Schedule a New Member Meeting
Meet with the Member Liaison, tour Yale Health Center, and learn about Yale Health.

Activate MyChart
Access appointments, test results, messaging, and your health record.

Establish Care with Your PCP
Schedule your first visit with:
Internal Medicine
Obstetrics & Gynecology
Pediatrics

Transfer Medical Records
Complete the appropriate forms and send records to Health Information Management department at yhmedicalrecords@yale.edu:
Request Access to Protected Health Information Form for each family member to have copies of your medical records sent to Yale Health.
Pediatric New Patient Initial Questionnaire for each child in your family. Please have form submitted prior to your child’s first appointment.
Designation of Patient Spokesperson gives an adult family member or friend authorization by the patient to discuss their medical information with the patient’s healthcare team for the purposes of coordination of care or payment for care.

Transfer Prescriptions
Ask your current clinician to send new prescriptions directly to Yale Health Pharmacy. Contact the Yale Health Pharmacy with any questions.

Update Insurance Information
Complete an Insurance Update Form if you or any of your dependents have an alternate insurance plan. Send this form to the Coordination of Benefits team at Yale Health Billing.

Download your Member ID Card
Download your member ID card for use during emergencies or when receiving care outside the Yale Health Center. You can upload your electronic ID to MyChart and share it with enrolled dependents.

Did You Know?
Yale Health is separate from Yale Medicine and Yale New Haven Health. Most Yale Health services are provided at the Yale Health Center, 55 Lock Street.
