Primary Care
Primary care is the foundation of your healthcare experience at Yale Health. It’s often your first stop for most health concerns. Over time, your primary care provider (PCP) gets to know you, your health history, and your healthcare goals, helping you make informed decisions at every stage of life.
Your primary care provider (PCP) is your main healthcare partner, helping you stay healthy, manage ongoing conditions, address new health concerns, and coordinate care throughout the Yale Health system. Because your care is coordinated within the Yale Health system, your primary care team can work closely with specialists, pharmacy, laboratory, diagnostic imaging, and other services to support seamless care.
What Does a Primary Care Provider Do?
Your PCP:
Gets to know your health history, goals, and healthcare needs
Provides preventive care, screenings, and routine checkups
Diagnoses and treats many common illnesses and conditions
Helps manage chronic health conditions
Answers questions about your health and medications
Coordinates specialty care when needed
We encourage you to contact your PCP first for most scheduled and non-urgent healthcare needs. We believe that developing a long-term relationship with your PCP leads to more personalized, efficient, and coordinated care.

Choosing Your PCP
Every Yale Health member is assigned a PCP upon enrollment, including children enrolled through Pediatrics.
Members who receive care through Obstetrics & Gynecology also have a PCP in that department, in addition to a PCP in Internal Medicine.
If you would like to choose a different PCP, contact Member Services at 203-432-0246 to review available providers.
