New Student Health Requirements

Welcome to Yale Health and Yale University!

This page provides information about your health requirements, submission and documentation deadlines, and Yale Health services. Please read this material carefully to ensure you understand the requirements and your obligations.

These requirements are mandated by Yale University policy and Connecticut State regulations. If you do not complete health requirements before your established deadline, you may be blocked from registering for classes in the future. You will need to upload all your health information to our system, Health On Track, for processing, review, and storage.

The Health On Track portal will be open for incoming Yale Summer Session, incoming Summer Term, or Early Start students after March 11 at Noon EST

The Health On Track portal will be open for incoming Fall students after April 1 at Noon EST.

You will get your requirements assigned after receiving a Yale NetID and Health On Track receives your information from the Banner system and from your program if part of an Early Start cohort .

Start the process at least two months before the deadline for your program (below). This should give you enough time to get any shots, tests or fix any paperwork problems. We suggest you book an appointment with your current healthcare provider early. Many providers are busy during the summer months, and the wait times may be long.

All current students with an open requirement should have access to Health On Track now.

Please note: Additional requirements may be assigned based on answers to required assessments in Health On Track. 

How to Satisfy Health Requirements (All Incoming Students)

Step 1:

  • Log-in to Health On Track when the portal opens for the term. Access will be available after admission and receiving your NetID.
  • Navigate to the Student Health Requirements – Launch Point
  • Review and confirm your identifying information and answer some questions about your program and your unique situation.

Step 2:

  • Search State Vaccine Registries. We will guide you through querying state vaccine registries. If you have any immunizations administered in the United States of America, this step may identify some or all of your required vaccines. Any available information will populate Yale forms to assist in resolving requirements. All states are not available or may have incomplete information. After completing this step, your Yale forms will be available for download.

Step 3:

  • Complete Online Questionnaires. Review and provide general information, complete online student health history (applicable for degree-seeking students and students with access to Yale Health) and the Yale TB Risk Assessment Questionnaire. You may be assigned additional requirements based on your answers.

Step 4:

  • Review the Student Medical Requirements Form to determine what additional documentation is needed. This document is populated in the My Documents tab of Health On Track
  • Gather any missing documentation and/or bring your forms to your Health Care Provider to complete.

Step 5:

  • Return to Health On Track and submit any documents or lab test results as directed.
  • Monitor your Yale.edu email address and check Health On Track to ensure all requirements are “Compliant.”

Please note deadlines in Health On Track reflect term due dates. Review below for programmatic deadlines not reflected in Health On Track.

  • Spring Term: December 15
  • Yale Summer Term: April 15 – *Due to the shortened compliance period for summer term, health holds with registration blocks will be in place immediately after the requirement due date passes.
    • Yale Summer Session (YSS)
      • Yale Summer Session Students
        • Session A – Document Submission – April 15; Compliance - May 1
        • Session B – Document Submission – May 22; Compliance - June 5
        • Incoming Yale Students - Document Submission – June 1; Compliance - June 12
    • Summer Term (not YSS)
      • Yale Graduate & Professional School Students (not in YSS): April 15
      • Yale School of Medicine: April 15
      • Yale School of Public Health: April 15
  • Early Start Term: June 1 – *Due to the shortened compliance period for early start term, health holds with registration blocks will be in place immediately after the requirement due date passes.
    • If you are a fall degree-seeking student scheduled on campus prior to August 1st and a member of one of the special programs identified below, you must meet the compliance deadline for the Early Start Term.
      • Yale College – First-Year Scholars at Yale (FSY)
      • Yale College – Athletics if on campus prior to August 1 (Note requires submission of separate forms from the Athletics documents).
      • Yale Graduate & Professional School Students (not in YSS)
      • School of Engineering – Personalized Medicine and Applied Engineering *Due to clinical activities, students are considered Health Professions Students.
      • Yale School of Architecture.
      • Yale School of Management – Master’s in Public Education Management
      • Yale School of Management – MBA for Executives
      • Yale School of Medicine – START Program
  • Fall Term: August 1

Important Instructions

  • Start Early! Documentation reviews can take up to 10-15 business days to review during the busy months of July & August.
  • Monitor your Yale.edu email address and check Health On Track regularly. Note: Communications from Health On Track are sent to your Yale.edu email address.
  • Ensure your name and date of birth are included on ALL pages of submitted documentation.
  • If you need additional assistance, please login to Health On Track and submit a request via the Contact Us tab.
  • Any current student with an open requirement should have access to Health On Track now.

Supplemental Forms for Certain Groups

Exemptions from Required Vaccinations

  • Medical exemptions: Medical exemptions may be requested by completing the Student Vaccine Exemption Form. Documentation from a healthcare provider is required and will be reviewed by Student Health for appropriateness.
  • Religious exemptions: In accordance with CT state statute, Yale does not offer religious exemptions for required vaccinations except for influenza. Under Public Act 21-6 passed by the Connecticut legislature, no new religious exemptions are allowed after April 27, 2021. The law outlines an exception for those with a valid religious exemption established by midnight April 27, 2021, for students enrolled at Yale on or before April 28, 2021. Religious exemptions from outside of Connecticut OR exemptions from K-12 schools will not transfer to Yale. Requests for religious exemption for COVID vaccination are available on Health On Track.

In order to receive care from Yale Health, your consent to treatment is required.  By presenting to the Yale Health Center for care, you implicitly consent to and/or acknowledge the following: (i) you consent to treatment as a patient of Yale Health for the purpose of receiving medical care and treatment and/or diagnostic procedures as determined to be necessary or advisable in your care; (ii) you consent to admission to the Yale Health Infirmary when indicated by your medical condition; (iii) if you are an undergraduate student, you consent to the performance of one or more tests to determine your blood alcohol breath level if you present to Yale Health and appear to be intoxicated, in connection with which you may be admitted to the Yale Health Infirmary or transferred to the Yale-New Haven Hospital emergency department; (iv) you acknowledge that Yale Health may use telehealth tools in your care including, but not limited to, video visits, e-consults with specialists, and audio and/or video monitoring in acute and inpatient departments; (v) you acknowledge that as part of your medical care and treatment, you may be tested for HIV and this testing is voluntary; you will notify your Yale Health care provider if you do not agree to HIV testing; and (vi) you acknowledge that photographic images, videotaped images or other images may be made of you for purposes of medical documentation or education as Yale Health deems appropriate, and that the use or release of such images will be in accordance with Yale Health’s Notice of Privacy Practices. You acknowledge that discussion of the risks, benefits, and alternatives to each procedure, treatment, or test is available to you so that you can make informed decisions about your care.
