Your Primary Care Provider (PCP):

Your PCP is the medical professional who knows you best and coordinates all aspects of your care. We believe a close relationship with your PCP is key to efficient and effective healthcare. Your PCP is part of a comprehensive clinical team that includes:

  • Physician Associates
  • Nurses
  • Medical Assistants
  • Clinical Receptionists

Our Collaborative Approach:

This team collaborates to provide the care you need, answer your questions between visits, and coordinate referrals to specialists both within and outside of Yale Health. By leveraging our shared electronic health record system, we ensure seamless communication and continuity of care across your healthcare journey.

Why See Your PCP:

We encourage you to see your PCP for most of your scheduled and unscheduled care. When it's not possible to see your PCP, other clinicians on your primary care team are available to assist you, ensuring that all your healthcare needs are met.
