Health Information Management

Our team of Health Information Management representatives manage and maintain the medical records of all Yale Health patients. They are available to answer questions regarding your release of information, authorized third-party requesters, and more.
Representatives can also assist with the completion of the Designation of Patient Spokesperson form and the Designation of Contact Information form.
To get a copy of your medical record, or submit health forms and other outside health information:
- Log in to MyChart and navigate to the Sharing Hub under the Main Menu. Select “Yale Health” from the list of providers. The information you need may be available for you to download yourself.
- If the necessary information is not accessible on MyChart, a valid authorization for the release of information form must be completed to obtain medical records for a third party. To access your own protected health information, complete a Request Access to Protected Health Information form. You may submit the form, or any outside health information to be added to your record in any of the following ways:
- In-person at Yale Health
- Via email to yhmedicalrecords@yale.edu
- By mail to Yale Health, P.O. Box 208237, New Haven, CT 06520-82327
- By fax to 203-436-5536
When requesting medical records, please note:
- Requests are generally processed in the order they are received. Allow 30 days for processing your medical records request.
- The following items can be released within 6 days: immunization records, prescription records, and the most current physical exam record.
- You can authorize someone else to pick up your records on your behalf if you provide a signed note giving them permission.
Medical records are kept for 7 years after you stop receiving care at Yale Health.
