Yale Health Center partners with Physical Therapy and Sports Medicine Centers (PTSMC) with offices throughout Connecticut. PTSMC provides all of our physical therapy services. Partnering with PTSMC provides our patients with high-quality physical therapy with more timely access and the convenience of multiple sites for appointments. Visit the PTSMC website for more information about their services and locations. Please note, Yale Health patients cannot use the “Request an Appointment” feature on their website. 

Students who have waived Yale Health Hospitalization/Specialty Care coverage and Medicare patients are not eligible for treatment in our Physical Therapy Department.

Appointment Information

For all new patients with Yale Health Plan: Please note, your physician’s referral can take up to 5 days. Once your referral is received, a Physical Therapy and Sports Medicine Centers associate will contact you to schedule your initial evaluation. If you have questions, please email YaleHealth@ptsmc.com or call (203) 498-5980.
