Yale Health Center partners with Physical Therapy and Sports Medicine Centers (PTSMC) with offices throughout Connecticut. PTSMC provides all of our physical therapy services. Partnering with PTSMC provides our patients with high-quality physical therapy with more timely access and the convenience of multiple sites for appointments.

Appointment Information
New Patients: Please be aware that your physician’s referral may take up to 5 days to process for new Yale Health patients. Once your referral is received, a Physical Therapy and Sports Medicine Centers associate will contact you to schedule your initial evaluation.
Scheduling: You will need a referral from a Yale Health clinician to schedule an appointment. After receiving your referral, a Physical Therapy and Sports Medicine Center associate will contact you.
Late Cancellation/No-Show Fee: A fee may apply for appointments canceled or rescheduled less than 24 hours in advance.
