Take Care of your Medical Care Before You Come to Campus

You should:

• Make sure you have completed the required health history form and make sure your pre-entrance immunizations are up-to-date and submitted.
• Bring original written prescriptions for medications you will need refilled while on campus, or have those prescriptions phoned in to the pharmacy 203-432-0033 by your current clinician.
• If you have a chronic illness or are presently being treated for a medical issue, please call Student Health to schedule an appointment with your clinician.
• Transfer your medical records to Yale Health using the Release of Health Information Form. Send them to Health Information Management / Yale Health / 55 Lock Street / Box 208237 / New Haven, CT 06520.
