Our team of registered nurses work together to ensure that members and students receive comprehensive, coordinated care during critical points of their healthcare journey. They facilitate smooth transitions and ensure appropriate and timely care for patients.
Transitional Care Management is designed to assist patients as they move from one healthcare setting to another. This team works to ensure that patients are receiving appropriate follow-up care. This includes:
- Hospital discharge coordination
- Post-discharge calls after hospitalization
- Coordinating various types of care after a hospitalization, including physical therapy or skilled nursing
Complex Care Management focuses on providing specialized care and support for high-risk patients and patients with chronic conditions that have complex medical needs. This includes:
- Supporting high-risk patients who are referred by their primary care provider
- Gender-affirming care coordination
