Patient & Family Council

The purpose of the Patient & Family Council is to be a resource for Yale Health in matters pertaining to the relationship between patients, patients’ families, and Yale Health staff and operations. The council members work together, at the request of Yale Health leadership, to promote, review and advise on operational processes and programs that address the needs of patients and families.

The council also facilitates discussion regarding health care services within the context of Yale Health and the greater health care environment. Patients and family members also have an advisory role in providing feedback on relevant projects undertaken by Yale Health.

Kelly ‘s volunteer experience includes weekly visits to Connecticut Hospice and fundraising for A Better Chance of Guilford.  She brings to The Health Patient & Family Council a background of financial service skills and project management experience, that will enable her to clearly assist the objectives of the Council. Kelly has a BA from Central Connecticut State University. Having benefited from Yale Health’s focus on preventive care in managing a chronic illness, Kelly hopes to find ways to encourage more patients to take advantage of preventative care.

Pamela is a co-chair of the Working Women’s Network Mentoring Committee. She holds a Bachelor of Science degree in Psychology and an MA. Ed. degree in Education. With a background in teaching, Pamela has always been passionate about mentoring students and channeling them to invigorating experiences in their personal and professional careers. Her interests lie in diversification of resources for patients and supporting their needs. Pam enjoys traveling, spending time with her family, and working on her first book.

Anna has a BA and MA from New York UNiversity and is a Chartered Financial Analyst (CFA). Anna’s past career in international investment management enabled her to travel around the globe, and meet people from different cultures and backgrounds. She looks forward to offering her perspective to foster growth and mutual understanding of the patient-provider relationship. Currently, Anna tutors for Literacy Volunteer and is a Taxaide volunteer for AARP. She enjoys spending time with family and friends. She loves planning the next family adventure and taking road trips to visit our beautiful national parks!

Fred has worked in clinical research administration and compliance at the Yale School of Medicine for the past 19 years. Presently, he is a senior research compliance auditor at Yale Medicine. His interests lie with asthma management as well as caregiver support for patients with chronic pulmonary diseases. Fred is passionate about mental health services for young adults and LGBTQ community issues. He has expertise with clinical research administration and data information systems (EPIC) and the tie-in with various mobile applications.

Nicole Evans

Nicole is a member of the Course of Study Committee (CSC), volunteers as a Building Ambassador for the Working Women’s Network, and enjoys participating in many of the events sponsored by Yale’s Affinity Groups. Nicole has a BS in Neuroscience from Trinity College. She is a former educator and vice-principal in the private school setting and continues her devotion as a literacy tutor for New Haven Reads. Nicole has over 12 years of experience and background working in healthcare at Hartford Hospital as a Laboratory Technician and Cancer Analyst. She is passionate about patients advocating on their own behalf, regarding their care and becoming active healthcare partners with their physicians. Nicole’s interests include spending time with her family, including their pet dog Sammie, exercising, reading, volunteering, and hiking.

Santo co-chairs both the Yale Veterans Network and the United Way Campaign for Human Resources. He has a background in management and communications as well as labor and industrial relations. He enjoys playing with his grandchildren and flying his hot air balloon. He is also a volunteer trolley operator at the Shoreline Trolley Museum and at Yale-New Haven Hospital.

In her current role, Beth focuses on identifying and improving patient safety issues surrounding transitions of care. Beth has a law degree (JD) and a Master of Public Health (MPH, Health Policy and Management) degree as well as a Bachelor of Science (Nursing) degree. She has chaired both the Domestic Violence Advocacy Council and University of North Carolina - Wilmington Nursing Alumni Association.

Grey Kupiec has been an employee at Yale for eleven years, serving in a variety of roles throughout the University. Since 2011, Grey has assisted the Office of Facilities with graphic design and information resources in support of Capital Project approval. Grey received his BA in Communications from the University of Connecticut, and his MA in Media Studies from The New School. Having grown up in Hamden, CT, Grey is a lifelong local of the New Haven area.

Prior to joining the Graduate School in 2017, Leah worked for nearly ten years in undergraduate admissions in Yale, where she served as Associate Director of Admission and an admissions committee chair. While in undergraduate admissions, she developed expertise in international admissions, project management, and technology implementation. Leah graduated with a B.A. from Yale College and received an M.B.A. from the University of Connecticut. Leah was a four-year letter winner on the Varsity Softball team as a Yale undergraduate. She and her husband are in the midst of raising two bright and exuberant toddlers with special needs. Leah is most interested in pediatrics, preventive healthcare, access to urgent care, customer service, and building trust through strong patient-provider relationships.

Theresa has a background working in healthcare with over 30 years at Yale School of Medicine including working to improve Press Ganey surveys for patient care. She is a member of the Linda K. Lorimer Award Committee, and a co-chair of the Workday Committee. She enjoys spending time with her family, reading, exercising, playing with her dogs and riding on the back of her husband’s Harley Davidson motorcycle. Theresa’s focus is on customer service, referrals to specialists, mental health and prescription management.

Colleen has been a Yale Health member since her husband began work in the Pathology Department in September 1997. In addition to being a wife and a mother of three children, all of whom have been raised in the Yale Health System, she is a speech and language pathologist in North Branford Public Schools.  She is grateful for the services and resources afforded to her as a member of Yale Health, and is excited to be a member of the Patient & Family Council. In her free time, she enjoys being with her family, cycling, reading, and being outdoors.

In her current role, her focus is on clinical affiliation agreements as well as ensuring compliance of students during their clinical experiences. Sandy has a bachelor’s in Education and a master’s in Industrial and Organizational Psychology. She has always had an interest in improving processes and systems and hopes her experience will be an asset to the council. Sandy enjoys spending time with her husband and two young daughters who keep her busy with flag football, swimming and drama. She also enjoys cooking and trying new recipes.
