Submitting Required Documentation: Health On Track
All health clearance items are now reviewed and completed through Health On Track. Please submit all required documentation via the Health On Track Portal (Net ID Required). Any clinical inquiries related to health clearance requirements must be submitted through that portal.
Health On Track is Yale’s enterprise system for uploading and tracking health requirements. Whether you are a student, employee, or faculty member, health requirements allow Yale to promote a safe environment for everyone who lives and/or works on campus.
Health requirements may include vaccinations, tests, or examinations and vary depending on your role at Yale. They may be related to safety or labor laws, or university policies. You may view your requirements through Health On Track, see how to complete them, and submit documentation as needed.
