Transitional Care Management
- Hospital discharge coordination
- Post-discharge calls
- Coordinate multiple types of home care after a hospitalization, such as skilled nursing and physical therapy
Complex Care Management
- Support high risk patients who are referred from their primary care provider
- Gender-affirming care coordination
- Monitor patients who are receiving care in skilled nursing facilities
