Yale Health Mail Order Pharmacy Service Instructions

Step 1: Eligibility

Determine if your prescription is eligible for mail order by going to the Yale Health Drug List. Drugs eligible for mail order will be listed in bold.

Step 2: Criteria

If your drug is eligible, you must also meet the following:

• The prescription was previously filled at the Yale Health Pharmacy and prescribed by a Yale Health physician.
• The prescription must be for a three month supply.
• Your physician must feel that your need for the prescription drug has reached a stable level where a prescription for 3 months at a time is appropriate.
• The shipping address must be a Connecticut address.
• You have 3 to 4 weeks of medication left from the previous fill. Please allow 3 weeks for processing and delivery.

Step 3: Fill out the Form

Print the Mail Order Enrollment Form or obtain the form in the lobby of 55 Lock Street. Follow the instructions on the form.

• How much is my co-pay for a prescription through mail order?
  Clerical & Technical (C&T) and Service & Maintenance (S&M) and YPBA staff pay 1 co-pay for a 90-100 day supply. See the chart on the Pharmacy Benefit page.

• The form asks for a security code on my credit card. Where do I find this number?

Notes:

Yale Health cannot be responsible for the carriers who provide the delivery service. If multiple attempts at delivery fail, the patient may be considered unsuitable for the service.

The form must be signed and dated and mailed or faxed to the address on the form.

Incomplete forms will be returned to the shipping address.

This mail order service is separate from the retail Yale Health Pharmacy. For all questions please call Yale Health Mail Order at 1-888-432-9427. Do not call the Pharmacy as the retail Pharmacy staff will not be able to assist you with your mail order questions.
