Health Information Services

Health Information Services is your resource for obtaining copies of your medical records or updating your Yale Health medical record with information from outside medical facilities.

Representatives are available to answer questions regarding requests for protected health information for members and authorized third party requestors.  They can also assist with the completion of the Designation of a Personal Representative to Access Health Information Form and to answer any questions.

A valid authorization for the release of information must include the following:

• An authorization is valid for six months (180 days) from the date the release is signed.  Response to requests will be within 30 days of the receipt of the request.

• Urgent requests will be addressed within 24 hours for medical emergencies.
